Evaluation Process

Once your evaluation has been scheduled, you will receive intake paperwork to complete prior to your appointment. Completing forms in advance allows us to use your appointment time efficiently and focus on your child’s needs.

During the evaluation, we will:

• Review developmental and medical history

• Discuss parent concerns and goals

• Conduct standardized and/or informal assessments

• Provide recommendations for services

A written report and treatment plan will follow the evaluation.
